Reduce ApoB
ApoB 80 mg/dL or lower. The single most consequential cardiovascular target.
What it does
Atherosclerosis is a cumulative-exposure problem, and ApoB is the particle count that drives it. Target under 80 mg/dL, under 60 if already at risk. Start with diet and body composition; if the number holds, escalate to medication without theatrics.
Each ApoB-containing lipoprotein carries one ApoB molecule — more particles means more chances for arterial-wall infiltration and plaque. Lowering particle number lowers lifetime exposure.
What the evidence shows
Overall evidence strength: High certainty
Who it's for, who should skip
Anyone optimizing cardiovascular risk. Diet and body composition first, retest in 3 months, then escalate to medication if the number holds. Waiting for ApoB to fall on its own is a common, expensive mistake.
Measuring it
Retest ApoB — 4–12 weeks after a meaningful change, then every 6–12 months — ApoB follows the lipid-panel schedule..
